Abstract
Well differentiated follicular thyroid tumors are a diagnostic, whereas anaplastic tumors are a therapeutic problem. The value of preoperative determination of thyroglobulin (Tg) in the serum for differential diagnosis of follicular tumors was evaluated in 516 patients. The Tg values were significantly higher in follicullar and Huerthle cell carcinomas then in other histologic entities including adenomas. A multivariate analysis of various factors in 154 patients with follicular carcinomas showed that tumor differentiation, primary tumor size, vascular invasion, distant or regional metastases, histological tumor type and age are independent prognostic factors connected with survival. In 9 patients with inoperablle or disseminated carcinomas the effect of low doses of vinblastine (Velbe) or adriamycine (Adria) were studied by DNA measurements and monitorig of morphological changes of tumor cells in fine needle aspiration biopsy samples of tumors. The results of these measurements were used for planning of combination of Velbe or Adria with radiation or chemotherapy with a sequence of Velbe infusions. The influence of Velbe on cell membrane fluidity was studied by electrone paramagnetic resonance (EPR) on cell cultures of Velbe sensitive and Velbe resistant cells. Velbe increased cell membrane fluidity of sensitive cells already at subtoxic levels whereas there was no effect on the cell membrane fluidity of resistant cells.
