Abstract
The coronary artery disease and heart failure as the and-stage of advanced coronary disease and most other cardiac diseases has become the most important health problem in Slovenia. They are not only the main cause of mortality but also represent an extremely high economical problem because of high mortality and associated expensive diagnostic and treatment procedures. With application of preventive measures in healthy people (primary prevention) and in already diseased subjects (secondary prevention) the consequences of both diseases can be effectively diminished. A lot of modifiable risk factors for coronary disease have been identified and it is possible to decrease the incidence of coronary events through their modification. However, numerous new risk factors have been discovered in recent years, among them are lipoprotein(a), homocistein and C-reactive protein.
The morbidity of heart failure patients can be diminished with early diagnosis and modern treatment. The gold standard for diagnosis of heart failure is still echocardiography. However, it is not very widely available and the new diagnostic methods are developping. One of them is the detection of increased blood levels of natriuretic peptides. The various new treatment modalities in heart failure and currently evaluating, one among them is the imunomodulating therapy.
In Slovenia there is a certain lag behind the developped countries in prevention of coronary heart disease and in the optimal managment of heart failure. The data on prevalence of various coronary risk factors and on their managment are still lacking. There are also no data on epidemiology and management of heart failure. With proposed research project we will investigate the prevalence of classical and new cardiovascular risk factors among adult slovenian population, the application of primary and secondary prevention of coronary heart disease, the hospital managment of heart failure patients, the role of natriuretic peptide in diagnosis and selection of therapy in heart failure and the effects of immunomodulating therapy in heart failure.
