Evidence shows that women with polycystic ovaries (PCO) are at increased risk of cardiovascular disease, yet mortality rates due to this risk are not higher. Ours is the first published study worldwide that provides evidence that PCO women with increased androgen concentrations have a shorter QT interval than those with normal androgen concentrations. Prolonged QT interval is an important risk factor for increased mortality due to cardiac disease. Increased androgen concentrations protect women with PCO against pathological processes manifested as a prolonged QT interval in the heart.

POF is frequently associated with autoimmune disorders. The presence of antithyroid and antiovarian antibodies together with abnormalities of the cellular immunity can potentionally represent an autoimmune mechanism of POF. The question of immunomodulatory therapy in selected patients with POF is open.

Premature ovarian failure (POF) and connected problems are usually underestimated by the therapists. Precise analysis of etiology and adequate treatment enable resumption of reproductive function in some groups of the women with POF. Therapeutic possibilities include estrogen replacement treatment in case of idiopathic etiology and corticosteroid administration in case of autoimmune etiology of POF.

The management of premature menopause permits a certain degree of success only after a thorough analysis of its causes. If autoimmune types (Hashimoto's thyroiditis, primary adrenal insufficiency – Addison's disease) are diagnosed and appropriately treated, the function of ovarian parenchyma can be restored. In other types of premature menopause, if the ovaries are not removed, it is possible to culture in in vitro conditions oocyte-like structures from the cells of the surface epithelium; this most recent discovery implies a possibility to develop oogenesis in vitro in near distant future.

In retrospective studies, statin therapy has been related to decreased incidence of sudden cardiac death (SCD) in heart failure. We sought to prospectively investigate a relation between atorvastatin therapy and SCD in patients with advanced chronic heart failure. We have found that atorvastatin therapy seems to be associated with decreased incidence of SCD in patients with advanced chronic heart failure.
