Significance for science
1 By using an experimental model of IVF/ICSI , we werr able to exclude other risk factors for infertility and for spontaneous abortion, like endometriosis, polycystic ovary, tubal factor and male infertility factor . This enabled us to proove an important negative role of a small ulterine septum in human reproduction as a negative factor on implantation , on pregnancy outcome and as a ectopic pregnancy risk factor. The results in assisted reproduction are consistent with the results in natural reproduction published by Pabuccu and Gomel in 2004, and Mollo i 2009, showing uterine septa as an important negative factor in human reproduction, not only causing spontaneous abortion and preterm birth, but also representing an infertility factors. The importance of our findings regarding small uterine septum has been shown by 34 citations in SCI journals . 2 Our data show that uterine septa may also represent an important risk factor for ectopic pregnancy . 3 We found an insignificant improvement in clinical pregnancy rates following ermbryo transfer using micronised progesteron for luteal supplementation in a natural and a modified natural cycle for endometrial preparation. 4 Our results showed that both, laparoscopic surgery and asiisted reproduction represent effective tools in the treatment of infertility due to endometriosis. According to our experience laparoscpic surgery should play a primary role in the complementary approach to the treatment of infertility due to endometriosis : by complementing both surgery and assisted reproduction 5 We found that womans age ) 39 years and a low ovarian reserve negativelly influnce the sucesss of IVF in the natural cycle 6) We found, that by morphologic evaluation of embryos on the day five, we can importantly improve our prognostic evaluation of IVF in poor respoders which enables us to differentiate between poor and bad response to ovarian stimulation . 7 We patented a teflon and metal made multiple use uterine manipulator, which enables us to better separate anatomical structers, to better identify vaginal fornices and to better close the vagina during endoscopic surgical procedures. -
Significance for the country
Having 2 million inhabitants and low fertility, Slovenia should be increasingly concerned about the reproductive health of its population. Data of the World Health Organization show that there are 8-10% of infertile couples worldwide, which means that in Slovenia there are 1000 new infertile couples each year. Preterm deliveries, representing 5% of all deliveries, are associated with 75% of perinatal mortality. In our research project, we studied laparoscopic, hysteroscopic and endosonographic procedures in relation with in vitro fertilization procedures in order to better define their effect on female reproductive health. The aim of this project was to contribute to improvement of diagnostic management of infertile couple, and to simplify the treatment of uterine and ovarian infertility factors and to prevent abortiveness and preterm deliveries with its perinatal complications . By analysing the effectiveness of endoscopic surgery and of assisted reproduction we contributed to the improvement of infertilitx treatment by following the hypothesis , that by precise endoscipic surgical procedures on ovaries and on uterus, we could improve both , the natural conception as well as the conception by assited reproduction techiques.. Our findings support the hypothesis . We not only improved and simplified diagnostic and therapeutic approaches to different infertility problems , but also reduced the problem of pregnancy losses and the problem of preterm brths. By introduction of the modified technique of laparoscipic cerclage, we importantly improoved reproductive prognosis in women with a high risk for pregnancy wastage and with a very high risk for a very preterm birth: With the use of laoaroscopic cerclege, we have also improved prognosis in 3 women after trachelectomy for an early invasive cervical cancer .
